Where can I find a generic model release form that is fill in the blank?

There was one that saved a photographer's bisquits back in 2006, very simple and plain:

I hereby release photographer and his or her legal representatives and assigns from all claims and liability relating to said photographs. All negatives and positives, together with the prints shall constitute your property, solely and completely.

Richmond Camera sells a pad of Model Releases. It's small enough to keep in your camera bag for just in case purposes and they're not expensive. Less than $10 if I remember correctly.
